IS IT A NORMAL LARYNX?: THE RANGE OF NORMAL IN VIDEOSTROBOSCOPY - Kost KM, Eibling DE
Objectives: 1. Review the basis of videostroboscopy. 2. Understand the spectrum of normal and the potential for misdiagnosis. 3. Learn the uses and limitations of instruments and scopes.
Abstract: Improved imaging capabilities of the larynx have parallelled an increased understanding of the mechanics of vocal fold movement. However, the wide variations in laryngeal symmetry, color, and movement may lead to difficulty in distinguishing what is normal from abnormal. The ability to make this distinction is important because of the implications with respect to investigations, potential misdiagnosis and unnecessary treatment. This workshop is aimed at residents and otolaryngologists wishing to increase their understanding of the spectrum of normal and integrate this information into their diagnostic skills. A large number of videoclips will be used to illustrate the transition from textbook normal to normal variants. Subtle abnormalities will also be reviewed. Data supporting the wide range of normal in videostroboscopy will be presented. Audience participation is welcome and will be encouraged.

This interactive workshop is geared to the otolaryngologist with an interest in voice disorders/laryngology. The panel will consist of specialists from multidisciplinary voice centres across Canada. Cases that pose difficult diagnostic and/or management issues will be presented. The cases will cover a wide spectrum of laryngeal pathology. Examples of behavioural, functional and structural voice pathology will be selected.
The presentations will include case histories and videostroboscopic evaluation and in some instances acoustic samples. The focus of this workshop will be to foster and encourage discussion between participants and the panelists.

CLINICIAN-PATIENT COMMUNICATION TO ENHANCE HEALTH OUTCOMES - Brown DH, Nanchoff-Glatt M
The average clinician performs 150,000 patient interviews during a medical career. However, many interviews are not conducted in ways that enhance health outcomes. Frequently, techniques are not used that can improve diagnostic accuracy, involved the patient in decision making and increase the likelihood of adherence to the desired regimen.
The challenge is to introduce techniques to clinicians and develop their skills in using these techniques in a brief period of time. To accomplish this, this workshop from the Bayer Institute employs three approaches. First, the workshop presents critical research findings that demonstrate the linkages between effective communication and enhanced health outcomes. Second, the workshop provides participants with an opportunity to analyze realistic videotaped interactions. The participant's task is to discriminate between those interactions that are effective and ineffective. Third, the workshop provides participants with an opportunity to practice techniques and receive feedback from their peers.

Drooling is a major handicap and constitutes a significant management problem in individuals with neurological disabilities of all ages. This workshop will present the major management options currently available.
The first speaker(SJ Daniel) will initially present all choices of treatment including oral motor therapy, biofeedback training, pharmacotherapy, and surgical options.
The operation of first choice in the authors` experience is submandibular duct relocation with simultaneous sublingual gland excision. A careful step-by-step approach and technical pitfalls will be presented by JJ Manoukian. Finally, the senior presenter (WS Crysdale) will detail the outcome and outline the expectations with this procedure.

OSSICULOPLASTY - SCIENCE AND PRACTICE - Bance M
Background:
Ossiculoplasty is a commonly performed operation. The results, however, are unpredictable. Many surgeons are passionate advocates of one technique.
There has been little examination of the biomechanics of ossicular reconstruction. The presenter is an experienced middle ear surgeon, and also runs a laboratory devoted to the biomechanics of hearing reconstruction. Insights from both perspectives will be presented.
Objectives:
To understand the biomechanics of vibration transmission in the normal middle ear
To understand how these are altered in the surgically reconstructed ear
To understand the impact of prosthesis tension, mass, shape, cartilage interposition and ancillary surgical techniques on the hearing results.
To demonstrate surgical techniques of ossicular reconstruction
Methods:
Using video and computer slides, options in ossiculoplasty, choice of prosthesis, effects of cartilage interposition, malleus versus tympanic membrane and size of cartilage cover will be discussed. Techniques to make ossiculoplasty easier as used by the presenter will be shown.

Course Description:
This course will present an integrated approach to mastering the structural and esthetic demands of modern tip rhinoplasty. Essential surgical anatomy and dynamics will be highlighted and the advantages of applying the tripod concept outlined. The various applications of vertical lobule division and tip grafting will be explored. Further refinement through skin soft tissue envelope manipulation and alar base narrowing will be described. The concepts presented will be illustrated through the use of long-term case presentations with dynamic diagrams and intra-operative video clips.
Course Objectives:
Review essential nasal tip anatomy and dynamics; provide an integrated, conceptual surgical approach to achieving superior tip rhinoplasty results; and illustrate the results of these technical maneuvers such that attendees can comfortably introduce them into their own rhinoplasty surgery practice.

BACTERIOLOGY OF CHRONIC SINUSITIS: IMPLICATIONS FOR THERAPY - Desrosiers MY, Woodham J, Bolger B, Witterick I, Frenkiel S, Kirkpatrick D
Bacteriology of chronic rhinosinusitis (CRS) has long been a matter of considerable debate. This has limited the development of treatment regimens specific to the bacteria encountered in this disease process. In the few reports available of the bacteriology of CRS, agents such as Staphylococcus aureus, gram-negative organisms, and coagulase-negative staphylococci predominate. Adaptation of treatment regimens designed for the treatment of acute maxillary sinusitis may thus have led to formulation and widespread adoption of sub-optimal treatment regimens.
In this workshop, we will explore the bacteriologic flora of CRS. A review of existing evidence will be presented then the results of a collaborative CSO-HNS Rhinology interest study group on the flora in CRS will be presented. A panel discussion will follow, with experienced participants discussing their experience and management strategies for problem cases.
It is hoped that this unbiased forum will lay the foundations for a more rational antibiotic management of CRS.

This course will present various options for managing dizzy patients. The emphasis will be on practical clinical management of dizzy patients, not research or esoteric theories. The moderator will introduce a topic and the other participants will indicate their philosophy regarding the issue. While it is unlikely that consensus will be seen the attendee will be exposed to a variety of options. The panelists will bring a few cases each for discussion. Audience interaction is strongly encouraged.
OBJECTIVES:
After this course the participant will be able to :
1. Discuss the current controversies in management of dizzy patients and 2. Utilize vestibular testing more effectively.

1. What is the optimal treatment of benign positional vertigo? Menieres disease? Post-traumatic dizziness? Non-specific dizziness?

LIP RECONSTRUCTION - Gullane PJ, Gilbert R, Audet N
This workshop will address all of the surgical options in the repair of lip defects following resection. The workshop content includes a brief review of the functional anatomy of the lip. Surgical techniques including rotation and transposition flaps will be discussed with particular emphasis on pratical surgical detail. The choice of flap, its limitations, complications and outcome in partial, subtotal and total lip reconstruction will be compared. Detailed schematic and intraoperative illustrations will be emphasized to provide a practical approach to the workshop participant. Long-term follow-up of each technique will be presented.
